Sys V Spooler Drops jobs

We're having troubles with the lp spooler dropping jobs. The job gets
spooled ok but never actually gets printed.

When I look in the /usr/spool/lp/request/xxxxx directory, there are
the d0-NNNN and r-NNNN files for the missing jobs. No errors get
reported from lp and the printers are all printing several hundred
other jobs each day. No jobs are report as being outstanding by lpstat.

Can anyone shed any light on this ?
