There's a very complete one available from research.att.com called,
surprisingly, F2C. It's in the /f2c directory, I believe. I think you
can FTP it. I got my copy via mail, since I don't have FTP capabilities.
I did a "send index from f2c" mailed to netlib at research.att.com.
