where is your ram and which bank holds what????
is bank 0 with 512k or is bank 1 with 512K
is bank ?? with 128K ??
you're missing some info(and so am I) but here's what I know

03 512K total (128K bank 0, 128K bank 1, 256K on exp board)

05 640K total (128K bank 0, 128K bank 1, 384K on exp board)
0? 640K total (128K bank 0, 512K bank 1)
0D 640K total (512K bank 0, 128K bank 1) ??????????
y = bank failed
1= bank 0 128K (for 64k dram )
2= bank 1 128K (for 64k dram )
3= bank 0 exp 128K (for 64k dram )
4= bank 1 exp 128K (for 64k dram )
5= bank 2 exp 128K (for 64k dram )
?= bank 1 row a (for 256K dram)
?= bank 1 row b (for 256K dram)
8= bank 1 row a (for 256K dram)
?= bank 1 row b (for 256K dram)
000 = segment failure
zzzz = offset
wwww = data written
rrrr = data read
Also note I think the 128K segmant map to existing ram regardless of conf.
in a 640K machine with 512K in bank1 an error of 5(128K,bank 2 expansion )
would map to the last row of memory bank 1 with the proper offset added in.
