adaptec 1542/ISC 2.2 controller errors

I have been getting some really strange controller errors
lately - referencing an absolute sector way out in never 
never land (1727066) - any ideas on what could be going on?

OS is 2.2 386/ix on a 386/25 with 8 megs of RAM.  The problems
just started recently -
