Looking for a package to handle forms

I need a package that will produce filled-in forms on an HP Laserjet III.
It will be given a description of a form, including text in different fonts
and lines/boxes/etc, and plain text data as output from a database listing,
and should "merge" them onto a single sheet of paper.

The form "description" doesn't have to be built graphically.  I have no problem
giving it commands like "BOX(upper-left-corner,lower-right-corner)".  Also,   
drawing graphics on the printer isn't required at all.  I can get what I need
with access to the scalable fonts and the line-drawing characters.

Ideally, I would be able to extract the plain text data by field names and
"place" it wherever I need within the form.  I can settle, though, for pre-
formatting the text so it will already be in the proper position on the page,
and could just overlay the form in the LaserJet's page buffer.

Does software exist that will take care of this for me?  If so, please point
me in the right direction.  I'm pretty new to the unix environment and don't
know my way around the available software yet.
