I strongly suspect the problem is that you were using TK50 tapes and did
not wait for the tape to come ready before typing yes.  One important
thing to note (this happened to me which is how I know) is that at least
on the DECStation 5000 SCSI bus, the TK50 actually appears to be ready
and then about 5 seconds later it starts to blink the green light again.
If you are too quick on the draw to type return, you fail to notice that
part and get just the error you get above.
