4.2 restore, huh?!

BSD 4.2, Vax 11/750, rm05, tu77, restore(8):
After the usual "bad controller munches rm05 pack", I reformatted
an rm05, rebuilt the filesystem and them attempted to restore
from a level 0 dump. (newfs; mount; cd ; restore r)
After 50-100 feet into the 2400' reel, restore rewinds the tape
and asks for the 2nd volume (this is a two tape level 0 dump).
Mystified, I put up the 2nd volume. Moments later it starts
spraying the console with "./<path name> not found on tape"
(or the equivalent).
Has anyone ever seen this? I tried a different tape drive (with
rrestore) and got the same result. Is this a bug? Could it be a
tape error?
I tried (mt rew; mt fsf) and the only file on the tape takes up
at least 90% of the tape, so it's not an EOF.
