As Mark pointed out, the diagnostic occurs when the temp file grows larger
than nroff can handle.  This occurs frequently when the user forgets or
mistypes a command to close off a section of text (e.g. a footnote).  Nroff
never sees the end of the "held" section and attempts to stuff the entire
document into the temp file.
