Reporting of disk errors

We have a PI running Irix 3.2. Recently the internal disk (with root, /usr and
primary swap partitions) started going flakey, and I noticed some problems with
disk error reporting:
Occasionally the system would crash with the message
"I/O error in swap"
but it didn't say WHICH BLOCK the I/O error occurred on. Is there
any way to find out?
Errors on other partitions are always reported in syslog as:
dks0d1s0 Media error ... Block #12345
That is, it always reports the error as occurring on partition 0,
even though many of them aren't.
How do you then translate the block number given into something you
can type into the addbb function of fx(1M)?
